Cancellation Policies
Monthly Membership dues will be charged on the same day each month that the membership began, unless cancellation notice is received.
You may cancel your membership at any time*. Please provide 5 business days notice to cancel your membership by calling or emailing our clinic. Late cancellations or no-shows will result in a charge for the full retail price of the appointment scheduled, or loss of membership treatment credit.
* There is a 3-month commitment for month-month memberships. If members choose to end membership before the required 3 months, completed services will be billed at non-membership prices, and clients will be billed for the full price of services used. Non-member rates can be found at Rates
If your monthly service has already been provided prior to the monthly scheduled withdrawal, service payment will be processed at the time of cancellation at non-discounted rates.
Services are valid from the 1st of each month to the 31st of each month, regardless of when your payment is processed.
Acupuncture treatment and add-on services can roll over to the next month. – Treatment credits expire after 60 days.
Monthly treatments may be gifted to new patients or family members only ($160 value)
Add-on services occur during your acupuncture services. They are not stand-alone appointments.
Refunds for unused membership credit:
- Within 30 days – 100% refund. After 30 days, no refunds are available.
- If payment is declined, a $25 fee may be assessed to the credit card or bank account on file.
- A bank account or credit card on file with Affinity Acupuncture & Rolfing™ is required for membership. Payments do not need to be applied to the credit card, but a credit card will be kept on file for unpaid services.
- If payment is not rendered for service provided, and the attempt for payment has not been resolved, we reserve the right to pursue legal action to recoup lost payment.
